PARTICIPANT AGREEMENT, RELEASE AND ASSUMPTION OF RISK FORM
This form must be filled out, signed and on file before any participation of activity at Reynard Raceway.
All minors must have a legal guardian PRESENT at registration to sign-up for practice and racing.
If the legal guardian will not be present at registration, the minor must be accompanied by an adult designated by the legal guardian(s) with the following form SIGNED. You will need to complete one form for each event. For example, you would need one copy of each form for a Saturday practice, and one copy of each form for the race on the following day.
The following form must be printed in COLOR, completed, notarized, and present at registration if the legal guardian(s) will not be present:
